In the event that we cannot reach you, please list an emergency contact for your child. 

I understand that by registering my child, I GIVE CONSENT to the following:

My child is physically and mentally capable of conducting age-appropriate science experiments under supervision. 

My child will be checked in by a parent/guardian and will receive a pick-up tag. Whoever picks up my child will have this tag or the tag number to show our registration team at the end of the day. No child will be checked out out to go home without this tag or the valid driver's license of the person who registered the child.

This is a private event that is offered for free as a service to the public. Pictures will be taken of this event and posted on our web pages and social media, but no personally identifying information will be made public. 

In the event of an illness, injury, or medical emergency during a church activity, the church may obtain or arrange appropriate medical care, including emergency medical services, when a parent, guardian, or emergency contact cannot be reached and/or the situation requires immediate medical action. Any medical expenses incurred are the responsibility of the participant and their legal guardian. I release The Rock Church from any liability that is not due to negligence.

I understand that parents will not be able to stay for this event. Every adult encountering our children must be background-checked and approved; therefore we will not allow parents to stay.
